Patient is admitted for laparoscopic bilateral tubal banding with falope rings for permanent sterilization. Assign both ICD-10-CM and CPT codes.

Patient is undergoing fertility testing. Patient has tried for 4 years to get pregnant. Today she is having a hysterosalpingogram to determine if her fallopian tubes are blocked. The physician inserted a catheter and injected contrast to view the fallopian tubes. Assign the appropriate code(s).
