Using the CPT manual, select a code for the procedure completed according to the following report:
  Preoperative and postoperative diagnosis: Nerve compression
 
  Procedure: Decompression of nerve
 
  The patient was prepped and draped in the usual fashion. The right foot was sterilized. An incision was made at the base of the foot, and the plantar digital nerve was identified. The tissue surrounding the nerve was dissected from the plantar digital nerve to release the pressure that was on the nerve. The incision was closed. The patient tolerated the procedure and was sent to the recovery room in stable condition.

  Procedure: Anesthetic injection into the axillary nerve
 
  Local anesthetic was placed into the axillary nerve via a syringe to obtain a nerve block. The patient reported pain at level 9 at the start of the procedure and then reported pain at level 4 following the injection.
