Describe how you have personally experienced the history of present illness during an E/M service visit (a visit to the physician's office, the ER, accompanying a child or parent during a visit to the physician.) Did you describe the HPI, if so how? Have you experienced the absence of any HPI questions? How did this impact your experience of the E/M service visit?

Answer to Question 1

Due to the personalized nature of this question, the student's long answer should include:
 The clinical setting of the E/M encounter and
 The chief complaint (reason for the visit)
 Description of the history of present illness of the CC
The student's long answer should also include:
 Whether the student has experienced a visit when the HPI was NOT addressed
 How the absence of the HPI impacted the student's experience of the E/M service, especially the quality of care received.

Answer to Question 2

Answer should include:
 Because every patient is unique, the documentation of the HPI improves helps the performing physician address the nature of the patient's CC. This allows the performing physician to treat each patient as a unique individual.
 It is important for physicians to understand the HPI because this is the physician's first interaction with the patient AND the CC. When the performing physician understands how those initial questions make a difference understanding the patient's CC, then the importance of documenting these initial questions becomes clear.
 If the HPI is collected by the performing physician but not documented, this could impact the future coordination of care received by the patient.
