Answer: The four elements are location of the burn, degree of severity of the burn, percentage of the total body burned, and percentage of the total body with third-degree burns.

Answer to Question 2

Answer: ICD-9-CM codes establish medical necessity; translate written terminology or descriptions into a universal, common language; and provide data for statistical analysis.
