The nurse is caring for a 4-year-old child who comes to the pediatrician's office with cold symptoms and appears pale. Vital signs are obtained and the child's blood pressure is 68/42 mmHg, a pulse of 98, and respirations of 20.
 
  The nurse is aware that: 1. The vital signs are normal in a child this age with cold symptoms.
   2. The respiratory rate is elevated and should be communicated to the physician immediately.
   3. The systolic blood pressure is too low and should be communicated to the physician.
   4. The pulse is too rapid and should be communicated to the physician.

Question 2

When treating chemical burns, the initial treatment should include:
 
  1. Washing the area with large amounts of water.
   2. Soaking the body part in cool water.
   3. Using ice on the burn.
   4. Applying antibiotic cream.

2. Soak for partial thickness burns with blistering, not for chemical burns.
3. Applying ice can cause additional injuries.
4. This can be done after the injury has been treated.
