An Apgar score of 1, on a scale of 0 to 2 for color is given when the body is pink but the extremities are blue. Flaccid tone qualifies as a 0 on the 0 to 2 scale under muscle tone. Flexed body qualifies as a 2 on the 0 to 2 scale under muscle tone. Heart rate of 128 is within normal limits for the neonatal heart rate and would qualify as a 2 on the 0 to 2 under heart rate.
