Answer to Question 1

B
Feedback
A This is an important nursing assessment, but does not take priority if the birth is
imminent.
B All options describe relevant intrapartum nursing assessments, but the focus
assessment has priority. If the maternal and fetal conditions are normal and birth
is not imminent, other assessments can be performed in an unhurried manner.
C This is an assessment that can occur later in the admission process if time
permits.
D This part of the assessment can occur later in the admission process if time
permits.

Answer to Question 2

C
Because of the immaturity of the fetal nervous system, respiratory depression can occur in the fetus prior to delivery and/or the neonate after delivery. The statement that all clients receiving narcotics within 2 hours of delivery are given the antagonist is not true. Hypotension in mothers is not a usual adverse effect of opioids and more likely a complication of supine positioning during labor. Narcotics can be metabolized, but not with efficiency, due to the immaturity of the neonate's systems.
