The APN would recognize softening of the cervix as Goodell's sign; normally the cervix is firm. Chadwick's sign is the color change from pink to bluish-purplish in the mucous membranes of the cervix, vagina, and vulva. Hegar's sign, usually evident at 6 to 8 weeks gestation, is the softening of the isthmus of the uterus, often referred to as the lower uterine segment. Ballottement occurs when the floating fetus rebounds against the examiner's fingers.
