Answer to Question 1

D
The best intervention is to assist them in developing new coping mechanisms for the transition due to changes in the family structure that childbearing families will experience. While assessing issues confronting the family, assisting the family to adapt to changing roles and functions, and identifying external resources are part of the nurse's responsibility, they are not the most important interventions.

Answer to Question 2

A
Hypersensitivity to DMPA may occur and should be assessed for prior to administering DMPA. Obtaining a urine hCG, checking the client's Pap smear results, and taking the client's vital signs are not necessary interventions with the use of DMPA.
