Answer to Question 1

ANS: A
Using plain water for cleansing, followed by application of A&D ointment or petroleum jelly to the penis to prevent the diaper from sticking to it, is appropriate care for circumcision. Prepackaged diaper wipes may contain chemicals that could cause pain or inflammation. Do not remove the yellow exudates because this is a normal healing process. If bleeding occurs, notify the health care provider immediately.

Answer to Question 2

ANS: C
The stools of a breastfed infant are soft and mustard yellow. Green, watery stools may indicate diarrhea and should be reported to the physician. Babies should void at least five times a day. Stools normally change from a greenish to yellowish color as the meconium stool is eliminated.
