Answer to Question 1

ANS: D
It is not uncommon to hear murmurs in infants less than 24 hours old. Hearing a murmur after 48 hours indicates a need for further investigation, and the health-care provider needs to be notified.

Answer to Question 2

ANS: C
Omphalitis is infection of the umbilical cord stump. The area around the base of the cord should be kept clean and dry. Cleaning the stump varies according to agency protocol. The other instructions are not related to omphalitis.

This is partially backed up by the fact that early grandfather clocks displayed IIII for the number 4 instead of IV. Early clockmakers apparently thought that the IIII balanced out the VIII (used for the number 8) on the clock face and that it just looked better.
