Amy Morgan is a home health nurse. Today is Amy's first visit to provide care for Alfred Richardson, a 76-year-old client who was just discharged following hip replacement surgery.

Amy is aware that Alfred is at risk of injury related to falls. Appropriate suggestions to prevent falls that Amy could suggest would be all of the following except:
  1. Install a grab rail in the bathtub and use nonskid mats in the tub and shower.
  2. Remove low-lying objects (coffee tables and extension cords) and excess clutter (stacks of books or magazines).
  3. Remove scatter rugs and small bathroom mats that can slide and repair loose or torn carpet.
  4. Use low-watt bulbs (40 watt or less) that produce minimal glare to illuminate steps and entrances.
  5. When getting out of bed, sit up, then wait a minute or two before trying to stand.
