Answer to Question 1

D
Feedback:
Nurses should plan interventions specific to the underlying cause. Caffeine may increase anxiety. The nurse should allow adequate time for conversations. It may be helpful for the elderly clients to discuss the source of their anxiety, and it is likely not possible for the anxious persons to avoid thinking about the source of their anxiety. Medication may not be appropriate to treat anxiety in many cases and may cause additional problems.
