Answer to Question 1

A
Feedback:
People of faith have long understood the value of prayer, and now growing research evidence supports the positive relationship between prayer and health and healing. One need not be an ordained clergy to hold a patient's hand and offer a prayer. Nurses who are not comfortable offering prayers themselves can ask coworkers to pray with and for their patients who so desire.

Answer to Question 2

D
Feedback:
The nurse would not want to discount an activity this woman has been undertaking, but would also want to emphasize that an assessment of the person's prior and current health status is important.
