Dilation of the RV can force the interventricular septum to move to the left. This phenomenon usually occurs when a high pressure (>15 cm H2O) is used and the patient is volume depleted.

Uneven ventilation is likely to occur with high inspiratory flow. If, for example, the right bron-chus is partially obstructed, most of the gas flow would go to the left lung because gas flow fol-lows the path of least resistance. Consequently, a larger volume enters the left lung, creating higher airway pressures in the left lung compared with the right.
