You are caring for a patient with an indwelling catheter. As the patient returns from the radiology department you note the collection bag is on the side rail. After assuring the patient is comfortably back in his bed you also note the securing device is gone. What should you do?

Question 2

You are assisting in the care of an infant that has not had a wet diaper for 6 hours. Is this acceptable? What should you do?

Answer to Question 1

Immediately reposition the collection bag below the level of the bladder and affix it to the bed in the proper fashion. Obtain and replace your facilities securing device. Notify the nurse of your observations and your actions.

Answer to Question 2

You should notify the nurse immediately. The infant should be having wet diapers every few hours, 6 to 20 times a day. Not making urine signals dehydration in an infant and is very serious.
