Answer to Question 1

D
Feedback:
The nurse is practicing reality therapy when comforting the client by saying, What has happened in the past cannot be changed. The goal of reality therapy is to help people face reality, reject irresponsible behavior, and learn new and more socially acceptable ways of behaving. Rational emotive therapy disputes irrational beliefs and accentuates rational and positive behaviors. One-to-one therapy encourages the clients to express concerns and feelings and to indicate how staff can be most helpful. In remotivation therapy, groups are structured so that participating clients discuss issues that are meaningful to them.
