Tetraology of Fallot is a cyanotic defect. Cyanotic defects may cause a right-to-left shunt, in which venous (deoxygenated) blood is mixed with arterial (oxygenated) blood within the heart and then circulated to the body. The total amount of blood sent to the tissues has a decreased amount of oxygen because the blood that is sent is only partially saturated with oxygen. ASD, VSD, and COA are acyanotic defects. Acyanotic defects have a left-to-right shunt, which sends oxygenated blood into the venous system.
