Answer to Question 1

C

Answer to Question 2

D
Feedback:
The nurse should place a small soft pad beneath the client's head to protect the head from injury. The nurse should avoid putting anything in the client's mouth during a seizure. Doing so can cause the client's teeth to break because of the force. During a seizure, the nurse should observe the client for respiratory depression and not increased respiratory rate. Elevating the head of the bed to 45 degrees is not necessary during a seizure.
