Answer to Question 1

D
Feedback:
When caring for a client with a sprained ankle, the nurse should apply an elastic bandage to immobilize and provide support to the affected area. An ice pack may be applied within 24 to 48 hours. A warm moist pack may be applied only after 24 to 48 hours. Sprains are treated by elevating the injured part, not by lowering it.

Answer to Question 2

A, D, E, F
Feedback:
Whatever system a facility uses, all MARs include the client's name, medication name (usually generic and trade name), dosage, administration route, and scheduled times. As needed (PRN) medications are also included in the MAR.
