Answer to Question 1

C
Feedback:
A large amount of fluid withdrawn (>1,000 mL) during paracentesis or thoracentesis can result in vasodilation and hypovolemia (decreased circulating fluid volume). These situations can cause syncope (temporary loss of consciousness, fainting) and shock. Parecentesis involves puncturing the abdominal cavity, not the lung.

Answer to Question 2

A
Feedback:
Washing the hands and putting gloves on both the hands is a correct step. It reduces the risk of infection from handling the specimen. Clamping the catheter for less than 15 minutes before collecting the specimen guarantees a fresh specimen. It need not be clamped for 20 minutes. After the procedure, the catheter must be unclamped to allow free urinary flow and to prevent urinary stasis. It should not be unclamped just before inserting the needle. The nurse should explain the procedure to the client before beginning the procedure and not soon after its completion.
