Answer to Question 1

D
Feedback:
To avoid an accidental needlestick, do not break or recap needles, or detach them from syringes. Most facilities use some type of safety syringes or have safety caps available.

Answer to Question 2

A
Feedback:
Urinary specific gravity of 1.005 is most likely for a client who is on a diuretic. A client who is on a diuretic voids large amounts of urine with a low urinary specific gravity of below 1.010 . Such values may also be seen in diabetes insipidus. A value of 1.010 or 1.020 suggests normal values for specific gravity of urine. A value of 1.030 indicates a high urinary specific gravity, seen in either dehydration or in cases of fluid retention (e.g., edema).

Although the Roman numeral for the number 4 has always been taught to have been "IV," according to historians, the ancient Romans probably used "IIII" most of the time. This is partially backed up by the fact that early grandfather clocks displayed IIII for the number 4 instead of IV. Early clockmakers apparently thought that the IIII balanced out the VIII (used for the number 8) on the clock face and that it just looked better.
