Medication administration records should never be brought into the client's room. The sign reviewing the kind of isolation should be on the outside of the door to alert the staff of what is needed to enter.

Urinary specific gravity of 1.005 is most likely for a client who is on a diuretic. A client who is on a diuretic voids large amounts of urine with a low urinary specific gravity of below 1.010 . Such values may also be seen in diabetes insipidus. A value of 1.010 or 1.020 suggests normal values for specific gravity of urine. A value of 1.030 indicates a high urinary specific gravity, which is seen in either dehydration or in cases of fluid retention (e.g., edema).
