Answer to Question 1

B
Feedback:
The nurse should inform the family caregivers with regard to caring for infectious wastes. The client need not be kept on complete bed rest or assisted with all activities of daily living unless the range of activities is compromised. HIV-infected individuals need not reduce the intake of salt in their diet.

Answer to Question 2

D
Feedback:
A buzzer located under the mattress warns the nurse if an unsteady client attempts to get out of bed. Providing a phone close to the client's bed will not warn the nurse of a client who is trying to get out of bed. The client who may wander away from the unit may wear a special armband alarm device, which warns the nursing staff if the person tries to leave the unit without permission. If the client is tied with a leather restraint, the client may feel hemmed in, and this may result in depression.
