Answer to Question 1

A
Feedback:
Many terms can be used to describe pain characteristics. Examples include the following: Character: The client may describe the character of pain with terms such as aching and burning, Duration: The client may describe the duration of pain as occasional and intermittent. Severity: The intensity or severity of pain may be described as mild, slight, moderate, severe, or excruciating. Associated factors: Associated sequelae (consequences) of unrelieved pain may include nausea and vomiting.

Answer to Question 2

D
Feedback:
Treatment of anaphylactic shock involves removal of the causative agent and administration of antihistamines to block the effects of histamine on the blood vessels, bronchioles, and gastrointestinal tract. In severe cases, epinephrine may be used to counteract the vasodilation that occurs. However, this is not the immediate intervention. An allergy test may be performed once the emergency is over if an allergen has not been identified. An ice pack to the site can slow absorption.
