Answer to Question 1

C
Feedback:
Deficient fluid volume is the main problem that the client is facing and should be specified as such in the diagnostic statement. Effect of drug therapy is the etiology of the problem, not the problem. Severe throat pain and Difficulty swallowing water are some of the signs and symptoms of this problem.

Answer to Question 2

A
Feedback:
As part of the nursing diagnosis process, the nurse should direct nursing interventions to meet the client's high-priority needs. Identifying the disease, studying the physiologic manifestations of the illness, and establishing the cause and nature of the illness are all aspects of a medical diagnosis, not a nursing diagnosis.
