Answer to Question 1

A
Feedback:
The nurse should instruct the client to eat a healthy diet, drink plenty of fluids daily, and limit exposure to the sun to help maintain healthy skin. Using herbal products on the skin is not the most appropriate instructions that the nurse should offer to help the client maintain healthy skin.

Answer to Question 2

C
Feedback:
Sunscreens are used to protect the skin from the sun's UV rays. They can assist in prevention of skin cancers caused by sun exposure. The SPF determines the amount of UV rays that reach the skin. Most often, SPF 15 or 30 is used. Apply sunscreen thoroughly and evenly; cover completely. Reapply after sweating or swimming.
