More institutions are using discharge planning conferences with discharge specialists, or case managers, who collaborate with the care team to plan for the needs of the patient after dis-charge. These conferences also address the interventions that will get the patient to the point of discharge in a safe and effective way. The RN's role is to ensure that the right people are in-volved in the sessions and that needed patient information is available so that the best plan of care can be designed.
