Discharge planning requires the RN to anticipate the needs of the patient beyond the health care facility. More institutions are using discharge planning conferences with discharge specialists, or case managers, who collaborate with the care team to plan for the needs of the patient after dis-charge. These conferences also address the interventions that will get the patient to the point of discharge in a safe and effective way. The RN's role is to ensure that the right people are in-volved in the sessions and that needed patient information is available so that the best plan of care can be designed.
