The licensed practical/vocational nurse (LP/VN) is caring for a group of clients and delegates tasks to the nursing assistant (NA). Which action by the NA requires the LP/VN to intervene immediately?

The LP/VN needs to intervene immediately when the NA is preparing the wrong client for the enema. The other actions necessitate action, but not immediate action because it does not directly affect the client. The LP/VN needs to discuss with the NA that storing additional linens in a client's room decreases supplies available that may be needed for other clients, and once the supplies are in the client's room they cannot be used for the care of another client. Taking extra time on a lunch break is not being a good follower, so team work principles need to be reinforced. There are alternative ways to deal with a failure to clock out in the system, so this is not a priority situation to be addressed.

No matter how informal, a job description outlines what an employer expects. Without this, the employer can arbitrarily make all sorts of job requirements. A written job description is necessary and serves as a sort of contract describing what the nurse will do in return for a salary. Outright refusal is rather negative and closes the door on this employer even if they do eventually write a job description. Deferring to a teacher's instructions does not demonstrate autonomy or independent thinking.
