A licensed practical nurse (LPN) has recently begun working in a nursing home that uses the functional nursing model.

The nurse recognizes that each part of the nursing team has a specific function or task to complete. The LPN was assigned to change all of the dressings on the wing. While caring for a client requiring a large wet-to-dry dressing change to debride the infected wound, the LPN noted that the client was screaming in pain when she removed the old dressing, and the wound looked worse than it did the other day. Which actions should the LPN provide next?
  A) Complete the dressing, chart the findings, and go on to change the next client's dressing.
  B) Provide the pain medication and then change the dressing, and chart the findings.
  C) Ask the medication nurse to provide pain medication before completing the dressing.
  D) Report the findings to the physician and change the dressing and document.

Question 2

  A) accept the client's deep-rooted values and beliefs and ask the client if she has a specific treatment or healer she would like to use.
  B) inform the client that the nurse must provide only what the physician prescribes.
  C) accept that the client is different, but ask the client to try American treatment ways.
  D) provide treatments from the client's culture as long as they don't interfere with the ones ordered by the physician.

Answer to Question 1

Ans: C
Feedback:
Functional nursing needs the team members to communicate with each other, in order to provide the best possible quality of care for the client. Since the client is exhibiting pain, the LPN functioning as the wound dressing nurse needs to ask the medication nurse to medicate the client before continuing with the dressing. Usually the medication nurse is the only one who has access to medications. Choices A and D ignore the client's pain and further fragment the care being delivered.

Answer to Question 2

Ans: A
Feedback:
Cultural practices stem from values and beliefs. The nurse should accept the client's cultural beliefs and practices and ask what specific treatments or if the client has a healer she would want to help to provide the yin and yang. The nurse does not respect the client's culture by informing her that she has to receive only what the physician orders. Stating that the nurse accepts the client's differences but asking the client to try American treatments provides the message that the other culture is not as good as the American culture. The nurse should inform the physician about the client's request, so the physician may talk with the client and provide treatments that do not interfere with cultural-based remedies.
