A licensed practical/vocational nurse has a cochlear implant and was instructed to not go inside or around magnetic resonance imaging (MRI).

While working for an agency at a local hospital, the nurse was assigned a client on a telemetry monitor who developed neurological symptoms and needed an MRI. The telemetry unit required the nurse to stay with the client during diagnostic testing to monitor the electrocardiogram and provide emergency interventions, if needed. The best action the nurse should take is to:
  A) inform the charge nurse that he has a cochlear implant, and ask if there is someone that can stay with the client during the MRI.
  B) ask the MRI technician to turn off the magnetic field so he can accompany the client into the MRI, and where he can stay while the client is being tested.
  C) ask an unlicensed care provider to accompany the client to the MRI since he has a cochlear implant and cannot be around the MRI.
  D) ask the client's family to accompany the client into the MRI while he stays in the waiting room, so the nurse can accompany the client to the MRI.

Answer to Question 1

Ans: A
Feedback:
The best action for the nurse to take considering the fact that he has a cochlear implant and cannot go inside or near the MRI is to inform the charge nurse. The charge nurse will find the best individual to accompany the client throughout the MRI. The instructions before taking a client inside of a MRI is to remove all metal objects and the oxygen tank since the magnetic will attract the objects, so it is not the best action, and asking the technician the question only delays the charge nurse to find a licensed individual to accompany the client. It is inappropriate to ask an unlicensed individual or the family to stay with the client because they are not trained on emergency procedures, if such a need should arise.

Answer to Question 2

Ans: C
Feedback:
Functional nursing is sometimes efficient, but it is a fragmented approach to client care. Clients often have a difficult time establishing a relationship with the nursing staff because so many members of the staff provide for their care. Clients are not obliged to identify their needs daily and accountability may or may not exist.
