Answer to Question 1

shoulders were not relaxed

Rationale: Failing to relax the shoulders will cause the clavicles to appear more vertical rather than horizontal.

Answer to Question 2

lateromedial

Rationale: With fingers and toes, the medial and lateral surfaces are referenced from anatomical position. In order to keep the 5th digit close to the image receptor, the hand and finger are rotated laterally, placing the lateral surface in contact with the image receptor, therefore creating a lateromedial projection.
