C5 and midway between the MSP and lateral margin

Rationale: The CR for the AP/PA oblique projections of the c-spine is directed to the level of C5 and between the margins of the MSP and the lateral margin on the upside of the patient.

Answer to Question 2

superior to the level of the EAM

Rationale: The central ray is directed 1-2 inches superior to the EAM (external auditory meatus) in order to include the vertex of the skull. The lateral skull must include the frontal bone, vertex, and occipital bone.
