Demineralization of bone and primary or metastatic malignant bone disease can spontaneously fracture a bone without stress. Pathologic fractures may require more than a simple fixation of the bone fragments.

Union of bones occurs when a hematoma occurs and then a callus is formed. Calcification happens after the callus occurs, then the bone resumes its preinjury strength, which is enhanced by stress and exercise.
