When using the closed method, a tiny nick is incised into the skin and a Veress spring-loaded needle is placed through the skin into the peritoneal cavity. The delivery of an expansion medium such as gas creates a working space.

When the open method is used, an incision is made in the skin and a trocar and sheath are placed in the cavity or space. Expansion medium can be placed through a port into this space. No Veress needle is used.
