Answer to Question 1

D
Assessment of readiness includes identifying the meaning of the transitional experience to the client. In the adolescent, for example, the transition from junior high to high school may be desirable or dreaded. Similarly, expectations about the transition may vary from those that are realistic to those that are extremely unrealistic. The adolescent whose expectations are unrealistic is poorly prepared for such a transition.

Answer to Question 2

B
When change a change in behavior patterns that is marked by withdrawal from usual activities, excessive sleeping, poor appetite, and sudden outbursts of anger is noted, depression may be the adolescent's problem and the adolescent may need professional assistance.
