It is quite rare for people in differing socioeconomic groups, let alone an employee speaking to his superior, to call each other by their first names. Furthermore, such a personal and informal interaction even among social equals would not occur nearly as quickly in other cultures as it does in American culture, where first names are often used upon initial introduction to others.
