Answer to Question 1

C
Psychotic patients often are preoccupied with internal stimuli and find it difficult to comprehend the words and actions of others. They may misinterpret both words and actions. To gain cooperation, use simple, explicit, concrete explanations and directions.

Answer to Question 2

D
Social and therapeutic relationships both involve the giving of information, emotional investment, and personal satisfaction. These aspects all have differences, but they are minor in comparison with the difference in responsibility that exists between social and therapeutic relationships. In the therapeutic relationship the nurse has both ethical and legal responsibilities to the patient; these responsibilities do not exist in the social relationship.

It is tightly attached to the skin below it, so it compensates for the increased area by wrinkling. This happens to the hands and feet because they have the thickest layer of dead keratin cells.
