Answer to Question 1

D
Promoting client safety is a priority concern. Explaining this to clients reduces anxiety and supports
the client's will to live. Option A: the environment cannot be made completely safe. Option B:
Although some clients can be trusted not to harm themselves or others, some cannot. Thus, to
maintain a safe milieu, harmful objects are available for use only under supervision. Option C is not
an accurate statement.

Answer to Question 2

A
Final evaluation of a client's progress is made on the basis of client satisfaction with the new health
status and the health care team's estimation of improvement. For a formerly delusional client to
admit to feeling comfortable and free of hassles denotes improvement in the client's condition.
The other options suggest that the client is in danger of relapse.
