Answer to Question 1

ANS: B
A long-term relationship is necessary to care for clients in the community because the time span of
care is lengthy. Because hospitalizations are currently so brief, establishing a short-term relationship
is all that can be expected. Options A, C, and D are interventions that would occur in the hospital
rather than in the community.

Answer to Question 2

ANS: D
Touch has various cultural and individual interpretations. Nurses should refrain from using touch
until an assessment can be made regarding the way in which the client will perceive touch. The other
options present prematurely drawn conclusions.
