Answer to Question 1

D
Effective listening improves the ability to meet client needs. Changing the environment to decrease distractions is a practice important to listening skills. Finishing client sentences, not clarifying messages, and avoiding taking notes are barriers to listening and may prevent the nurse from picking up hidden messages, which could minimizing minimize misunderstandings if perceived.

Answer to Question 2

B
False imprisonment involves holding a competent person against his or her will. Actual force is not a requirement of false imprisonment. The individual needs only to be placed in fear of imprisonment by someone who has the ability to carry out the threat. If a patient is not competent (confused), the nurse should act with beneficence. Patients admitted involuntarily should not be allowed to leave without permission of the treatment team.
