Answer to Question 1

B
Noncompliance occurs with many individuals with mental health disorders because of the ways the side effects of the medication affect an individual as well as other factors. It is important to work with clients to prevent noncompliance. Depression, anxiety disorder, and obsessive-compulsive tendencies are not indicated in the situation described.

Answer to Question 2

D
The patient is at risk for depression due to having a chronic, deteriorating illness and resulting accumulating losses. She also displays hopelessness. These factors make the need for a thorough assessment for depression and suicide essential. Depression that goes unrecognized and untreated is often responsible for worsening the medical condition and diminishing functional abilities. The other options are not priorities and/or nontherapeutic.
