Answer to Question 1

A
Teaching about symptoms of anxiety, their relation to precipitating stressors, and the positive effects of anxiety will serve to reassure the patient. Mild anxiety, as suggested by the symptoms noted here, is not pathological and is rarely disruptive to functioning. It can serve to sharpen perceptual focus and lead a person to attend to important matters at hand. Medications and psychotherapy are important treatment modalities of more severe forms of anxiety, especially for disabling anxiety disorders such as panic disorder with agoraphobia, but are not indicated for mild anxiety.

Answer to Question 2

B
The patient mentions that her symptoms make her more interesting to people, and others were not so interested before she became blind, inferring that she is uninteresting and unpopular without the symptoms and supporting the nursing diagnosis of chronic low self-esteem. Defining characteristics for the other nursing diagnoses are not present in the scenario.

Your skin wrinkles if you stay in the bathtub a long time because the outermost layer of skin (which consists of dead keratin) swells when it absorbs water. It is tightly attached to the skin below it, so it compensates for the increased area by wrinkling. This happens to the hands and feet because they have the thickest layer of dead keratin cells.
