Answer to Question 1

2
Rationale: The anxiety is so disabling that these clients' functioning is adversely affected, but they may not be totally dysfunctional. The anxiety is a subjective feeling that is threatening and not objective.

Answer to Question 2

2
Rationale: Asking a question that helps the client focus on her feelings conveys the nurse's willingness to be with the client and understand the client's inner experience. Telling the client what would be best is giving advice. Asking the client why she would give up the baby does not convey the nurse's interest in the client's experience and limits therapeutic communication. Telling the client that she will feel guilty assumes an understanding of the client's feelings before the nurse has taken the time to explore the client's experience.
