Answer to Question 1

1
Rationale: When members are unable or unwilling to perform assigned roles, the family experiences stress. For the health of the family systemnot only family members but also their relationships, their communication with one another, and their interactions with the environmentroles often must be negotiated in other than stereotyped ways. When the roles are not negotiated satisfactorily, family disequilibrium results. Not following assigned family roles does not result in recognition, communication, advocacy, or acceptance.

Answer to Question 2

1
Rationale: The nurse is obligated to share with the client the limits of confidentiality in their exchanges. Information gathering and sharing are part of the mental health nurse's role and the expectation is that the nurse will accurately portray and convey data about the client. The nurse would not keep information from the rest of the mental health team.
