A 19-year-old female is concerned that she may have been exposed to the HIV virus by her boy-friend. For months she has thought about the possibility and the need to be tested.

Based on current findings, the nurse can hypothesize that the primary reason she has not been tested is because she:
  1. Distrusted the confidentiality of the health care system
  2. Was concerned about losing her current boyfriend
  3. Waited to see whether symptoms would develop
  4. Feared she might test seropositive

Question 2

Which statement made by a client with AIDS indicates that the client is exhibiting symptoms associated with acute psychotic-type dementia?

  2. I don't enjoy being with others anymore.
  3. I'd like to stop the voices I hear in my head.
  4. I can't always remember where I put things.

Answer to Question 1

ANS: 4
Fear of testing seropositive is a strong deterrent to being tested. As long as one has no concrete evidence, denial can be protective. Option 1 may be a factor for some clients, but this is probably secondary to the fear of learning that one is HIV-positive. Options 2 and 3 are not primary rea-sons.

Answer to Question 2

ANS: 3
Hallucinations, dementia, psychomotor agitation, and other psychotic behaviors are part of acute psychotic presentation of dementia associated with AIDS. The other options are more consistent with dementia chiefly characterized by mild depression.
