Answer to Question 1

2. National safety goals seek solutions to safety problems..

Rationale:
National Patient Safety Goals are focused on solutions to safety issues and prevention of further injuries. Instead of focusing on the person who made the error, the goals focus on finding ways to prevent that error from happening again. The staff members should not need to be reminded about safety as safety should be the culture of health care. Health care agencies want the public to know about their safety promotions, but that is not the goal of the program.

Answer to Question 2

1. Self-esteem.

Rationale:
The client is critical of her mother, verbalizes feelings of helplessness by saying that she does whatever her mother wants her to do, and is critical of self by saying she cannot do anything right. These findings help the nurse assess the client's self-esteem. Role performance would be assessed by asking the client about the different roles held and the satisfaction with each of the roles. Body image would be assessed by asking the client how she feels about her appearance. Personal identity would be assessed by asking the client to describe personal characteristics and self-concept.
