A client is experiencing a multisystem fluid volume deficit after hemodialysis. The nurse assesses the symptoms present as tachycardia; pale, cool skin; and decreased urine output.

The nurse determines that the client has not met the expected outcome for a client on hemodialysis, which would be to experience: 1. A reduction of extracellular fluid.
   2. The pharmacological effects of a diuretic infused in the dialysate.
   3. The effects of rapidly infused intravenous fluids.
   4. Cardiac failure.

Question 2

The nursing manager asks a nurse to participate in data collecting for the unit. The nurse cooperates in order to:

  1. Advance the nurse's practice.
   2. Participate in the quality improvement process.
   3. Prevent problems from arising in the unit.
   4. Fulfill legal requirements.

Answer to Question 1

1. Experience a reduction of extracellular fluid.

Rationale:
The client on hemodialysis is expected to have a reduction of extracellular fluid, not a fluid deficit that puts the client at risk. Diuretics are not administered during hemodialysis nor are IV fluids. Cardiac failure would not be an expected outcome of treatment.

Answer to Question 2

2. Participate in the quality improvement process.

Rationale:
Quality improvement is the name for the processes used by an agency to measure and improve aspects of client care. The nurse may advance practice, but that is usually accomplished by returning to school for a higher degree. Preventing problems from arising is only one benefit of quality management. Nurses are encouraged to participate in quality improvement programs but are not legally required to do so.
