Answer to Question 1

3. Obtain educational materials regarding the disease process and smoking cessation written in Spanish

Rationale:
There is a high rate of cigarette smoking and other tobacco use among the Hispanic population in the U.S. The intervention that would be most beneficial for the client would be for the nurse to obtain educational materials regarding the disease process and smoking cessation written in Spanish, so that the client may use it to refer to in the future. The client may or may not be able to tolerate activity. Obtaining a Spanish-speaking interpreter would be more beneficial for the nurse than the client. Encouraging family members to stay to assist with communication might be beneficial in the short term; however, it will not address the client's need to learn about the disease process and ways to stop the progression.

Answer to Question 2

1. Adjust the temperature in the room to a comfortable level
4. Limit the use of alcohol to early in the evening
5. Do not smoke before bedtime

Rationale: A comfortable room temperature will promote sleep. Alcohol interferes with REM sleep and should be consumed well before bedtime. Nicotine is a stimulant and may prevent the client from falling asleep. Exercise within a few hours of bedtime acts as a stimulant and can cause the client to be unable to sleep. Tea contains caffeine, which is a stimulant. The nurse could suggest noncaffeinated tea before bedtime unless this causes the client to wake during the night to urinate.
